Dr. M.H. Shaheel Mahmood is a senior consultant. ENT and head neck surgery. He probably has one of those heavy black chairs in his room. The ones that recline all the way back. People come in with swollen necks. Thyroid lumps mostly. Or voices that went hoarse months ago. He just sits there. Puts on his headlamp. The bright light hits the back of their throat. He uses a small angled mirror. Warms it up over a spirit lamp first so it does not fog up. Just basic physics. He asks them to say 'ee'. They always sound weird doing it. He writes down some notes. Maybe orders a needle test. Just scribbles on the pad.

Head and neck surgeries take a long time. Up in the OT. He cuts out thyroid glands. Or swollen lymph nodes. Lots of blood vessels in the neck. He has to be careful. Probably stands there for four hours straight. His own neck probably hurts from looking down so much. The nurses keep handing him the suction tube. It makes a loud slurping noise. He ties off the small veins with thin black thread. Sort of tedious work. He puts a plastic drain tube in before closing the skin. Just to catch the extra fluid. He tapes it to their shoulder. Tells the relatives outside it is done. They ask when the patient can eat. He says maybe tomorrow. Just cold liquids. He washes his hands. Dries them.
